Service/program name: Active For Life
Description: Active For Life is a flexible 10-week program that encourages people to be more active on a regular basis and is based at the work place. The program encourages people to set their own goals based on how active they are, and how active they want to be. The program requires support of management and staff. Each Active For Life participant is a member of a team led by a team captain who provides support to help members reach their goals. Throughout the 10-week program, activities and workshops help inform and motivate participants.

Description: The Cancer Survivors Network "is a community created by and for survivors and caregivers as an outlet to cope with the challenges and spectrum of emotions that living with cancer can bring." Description: Generation Fit gives youth the opportunity to "strengthen community actions skills such as planning, advocacy, and communications and participate in projects that can increase physical activity and healthy eating among their friends and family, and in their schools and communities." "The projects take about six to eight 30 minute sessions to complete, but they can be easily adapted to fit the needs and abilities of the group."

Description: Volunteer cosmetologists (all certified professionals) teach women undergoing cancer treatment "how to enhance their appearance using make-up techniques. Each woman learns to perform her own make-over using complimentary cosmetics and skin care products donated for use by the cosmetic industry for use during the program and later at home.

Description: The ACS's Prostate Cancer Education and Support Program helps men and their families cope with prostate cancer. Participation in the program provides information and peer support to assist in the recovery process. No medical advice is offered. The service provides personal 1-on-1 visits and telephone support, Man To Man Newsletter, and a world wide web sub-site.

Description: Reach To Recovery uses carefully selected and trained volunteers who have fully adjusted to their breast cancer surgery and treatment. A patient and volunteer may meet face to face or by telephone. Volunteers are able to provide support and up-to-date information, including literature for loved ones and friends. "They can even provide a temporary breast form and information on types of permanent prostheses, when appropriate.

Description: Tell A Friend is a program designed to get women 40 or older to have regular mammograms. Trained volunteers identify women they know who may need a mammogram and provide them with information and assistance to get one. The pilot study resulted in a 40% relative increase in mammography usage by women participating in the program.
