Service/program name: Case Management Services
Description: Offering frail, older adults and their families assistance as they assess the need for care and coordinate an overall plan of services to maximize the older person's independence in the home. Areas examined include medical care and supervision; home maintenance; personal care assistance; emergency response systems; financial needs; and living arrangements.
Caregive Program provides information, advocacy and services to caregivers and grandparents raising grandchildren that help them adjust to caregiving roles and fill gaps in services or resources. Services include caregiver support groups, educational opportunities, respite, counseling and limited financial assistance.

Service/program name: Horizons Transitional Housing
Description: Providing subsidized housing and supportive services to homeless families and their children while they work toward permanent housing and a quality of life that supports their needs. The program represents a unique partnership which joins the participating family with the professional case management services of Catholic Charities.

Service/program name: Adult Protective Services
Description: Offers trained and caring professionals to help determine the services most needed to stop neglect and abuse; protect the person who has suffered abuse and/or neglect; and assist the individual in the healing process. Intervention for the abuser is also provided.

Description: Providing information and referral to other community resources; food and travel vouchers; clothing and furniture vouchers; financial assistance for prescriptions; and rent/mortgage assistance. All available on a limited basis.

Description: Providing information about the many services and benefits available to older adults. Client advocacy and intervention are also offered.

Service/program name: Money Management Program
Description: Volunteers, specially trained and screened assist senior adults with their basic money management needs. Volunteer tasks may include developing an appropriate budget, sorting mail, bill paying, and balancing a checkbook. Two levels of services are provided: bill payer and representative payee. Volunteer activity and accounts are monitored on a regular basis and limited client fund insurance is provided by AARP.

Service/program name: Senior Companion Program
Description: Offering older adults, age 60 and over, the opportunity to support their homebound, frail peers through regularly scheduled home visits. Senior Companions earn a modest tax-free stipend and generally volunteer 20 hours per week. Annual recognition event.

Description: A full-time bilingual social worker is available to serve and advocate with the Hispanic/Latino community.

Description: In addition to abuse, neglect, and financial exploitation reports, we also take reports of self-neglect for individuals 60 and older and persons with disabilities (ages 18 59).
